Contact Management with site...

I have a need on my site (which is hosted here and includes WebWizForum) for some kind of a contact management form for members where a member of the site's community can go in, under password, and edit or add their own personal contact information beyond what the forum profile looks for.  I need stuff like mailing address, phone numbers, etc.  I'm OK with linking to it outside the forum, but it would be ideal if it were more integrated.  I know that I could probably import email addys into NewsPad, so that might be a way to notify people and still have those notice tie backwards into the forum.

I believe I can put this together as a seperate .asp page, but thought I would ask around to see if someone has seen something that's more "turn-key" out there.
