How to create a poll and select multiple options?

   Good Day,
I'm trying to create a voting poll where members would be allowed to only vote once. The catch is that I want the user to be able to make multiple selections.

I'm trying to create a trail work day sign up poll where I would propose serveral dates for trail maintance. Then the members would be able to select the dates they are avaible.

I then need to have the system automaticly total the members per date.

I would also like to be able to get a list of members most avaible on a date and email those people from the voting poll.

Is there a way to configure a poll to track who voted for what
and allow for multiple selections in a poll but only allow a member to vote once?

It would also be nice to be able to display the results on the front page of the website.

Would this function exist in the forums or would I need to code it?
