I had to do a manual DB install since GoDaddy will not allow outside scripting to be done on their SQL server.
 
All of the tables, relationships and constraints are created as well as inserting the initial data.
 
How do I get rid of the "setup" page and start using the forum?

Just remove the comment tag for SQLServer in database/database_settings.asp
