you can use CSS, but its not a guarenteed solution (there isn't going to be a "for sure" solution)

There are several components which allow you to "print" in a pdf or word doc. The problem is that they cost and not many hosters would install it. To sort a same problem I was forced to lear some php as it has some inherited clases that alow you to alter online and pdf document. I don't remember the one of word doc, but here it is the pdf component.

The simplest way is to have the form as a jpeg and use ASPJpeg to write on it.
