in line 74, if the database is mySQL, exist a error:
'Initalise the strSQL variable with an SQL statement to query the database
strSQL = "SELECT TOP 1 " & strDbTable & "Members.* " & _
"FROM " & strDbTable & "Members " & _
"WHERE " & strDbTable & "Members.ID_Code = '" & strUserID & "';"

change for

strSQL = "SELECT " & strDbTable & "Members.* " & _
"FROM " & strDbTable & "Members " & _
"WHERE " & strDbTable & "Members.ID_Code = '" & strUserID & " LIMIT 1;"
