Just want to ask why duplicate record can happen. I mean even I click the submit button once, it can still be duplicated and sometimes it happen and sometimes not. Is it the database problem?

Thanks for answering.

Is this your app doing it or what?

If its making duplicate records then your probably clicking submit twice or your refreshing your page or your adding it twice in the SQL statement.

I didn't submit it twice nor refresh it nor adding it twice in sql. Some said that it is because of the javascript method onclick(). I'm still looking for more possibilities. Anyway, thanks.

I encountered a bizarre situation that may be similar... it didn't have to do with inserts, just with selects. With a specific record (i.e. ONE (1) specific record - always) that existed in the DB (MS SQL Server) only once. When it was output, it would appear twice... I was too damn lazy to find out why though...

Here's another one..if you created the database yourself it could be that you didn't auto number the fields, or records.
