I remember seeing a mod once where you could let your members write their location instead of the drop down... Is this still possible?
 
Also, If I wanted to change the list to include states instead of countries where would I do this?
 
I would like to see a mod that would allow a user to Type in their City and select a state from the drop down... is that possible?

Just replace the drop down with a text box that has the same name/id.

Just add the text box. In the form handler, you'd have to concatenate the value of the text box and the drop down.
