How exactly does open source software like firefox operate?

Does everyone around the world submit snippets, then a pannel decides what goes in?

There are different types of Open Source Projects, most seem to have a dedicated group of developers (like WWF for example) but publish the source for you to change or customize. Some though are open where you can submit code changes and like you said a Panel decides which changes make it.
