Hello - I installed the form following the instructions but I keep getting an HTTP Error 403 - you are not authorized to view this page error. I am installing it in a hosted environment. Does the ISP need to help me with the install?

It does sound like you need to contact your web host as it is a server generated error.

Also make sure the web server uses the Windows Server Operating system with ASP enabled
