I am being required to submit the security code, but the application is not recognizing the code. Yes - I'm entering it correctly. Any help?

Here is the error message:

Error

The Security Code entered did not match that displayed.
A new Security Code has been generated.

More than 3 failed login attempts have been made on this account.
Please enter your details again, including security code.

I do love to repeat myself on a daily basis so here goes again:-
