I've tried and fail several times using this template. I tried to use it in wwf and I received the same results. http://www.webwiz.net/asp/tutorials/add_to_database_pt2.asp I create a forum - db and fields. http://www.mbd2.com/guestbook_form.htm I enter the data and noting is added/updated to the database... Seem to work for me.
Edit: I did noitce that the comment field is not being added to the database. So are you adding this T6 field to your database?

Yelp, I am in it three times. Like I said the comments are not being added. Also after you are redirected to the guest book page, the page is not order right. You should add a ORDER BY in your query so that the newest records are the first displayed. As it is now I think the oldest records are displayed first.
You may also want to setup an admin page so that you can delete those old records to clean up the database after you get your scripting down.
