Where is the On-line database creation script for SQL Server 7.0/2000 version located, I downloaded this version but did not see the script and cannot find it on the site.

it is in the root of your forum once you copied/uploaded the files.
It is called msSQL_server_setup.asp

I have searched the root and all folders and do not see that file.

You must have downloaded the wrong version, try re-downloading the forum again.
