I had replaced the icons to the left of forum subject. originally the blue diamond would have a flag or star to point out there has been a reply

How do I add a new message icon like the star used with default Icon to replace icon?

This is presently not possible.

If you wish to have flags displayed to indicate new posts or other activities within the forum you would need to use the default images, or replace the images in the forum_images folder with an image of the exact same size and type.
