I'm sure this isn't a problem with the app, rather a mental block on my part.  Either way, help would be appreciated.

I've set up a number of Categories in my forum, and want a specific non-ladder group (Contributor) to be the only ones with access to specific categories.

I've set those categories up with generic forum permissions of Private across the board.  I then go into Group Permissions Admin, select the first forum, and add permissions to allow Contributor access to read, write, etc.

The problem arises when I go to the next forum and attempt to add Group Permissions.  I Create New Forum Permissions for User Group, select Contributor, check the boxes and get:

Microsoft JET Database Engine error '80040e21'

The changes you requested to the table were not successful because they would create duplicate values in the index, primary key, or relationship. Change the data in the field or fields that contain duplicate data, remove the index, or redefine the index to permit duplicate entries and try again.

/forum/admin/create_group_permissions.asp, line 128

when I click the submit button.  It doesn't appear to allow me give Contributor access to more than one forum.

Thoughts?

Some upgrade scripts don't work correctly, try using the original fresh database if you can, also the database may have become corrupted, starting from scratch may solve the problem as error handling should mean that this error should never happen.

Is there anything manually I can look at to try to resolve it?  I used St8dog's script for the conversion, and this is the only thing that's cropped up and bit me.

I also tried the DB conversion script in your DL section, but after a successful upgrade, it won't let admin in.. it tells me that only users with appropriate access are allowed.. lol

A fresh install or sticking with the old version maybe your only option.
