Why doesn't the code of a smiley change in the smiley pic??

You need to make sure smilies are enabled in the admin section and smilie codes only work in the non WYSIWYG editor.

If you are using version 7 make sure that the 'Enable Forum Codes' box is checked at the bottom of your posts.
