The problems with banning using cookies is that cookies can be removed
from a users machine, in fact it's simpler to get round that having to
sign-up again.
If you enable email activation, then you can also bann email domains
from signing up, also you can not sign up with the same email address
as a suspended account, this would make re-signing up much more
difficult.

I tried this, but the system then offers them a resend of the activation email. Is that correct, or and anomally I'm seeing? If they can just get an activation email again, then the system itself seems to have a way for them to get around being suspended.

A suspended account can not be re-activated via a activation email.
